You will need to complete a HR Form if you need to amend any of the following:
Amendments to working schedule ie. 5/5 & 5/7
Change of work location
Adding or removing allowances
Contract renewals/extensions
Change of positions numbers
Change to employee Terms & Conditions
Leaving forms.
